Only One Category Request Per Form allowed. You can submit as many Category Request Forms as you feel necessary.

Note: There is a Fee of $10.00 per Category Request Approved and Programmed when not included as part of a special USADI offer.

The above fee also includes receiving and uploading of the 90 pixel x 90 pixel  (Web Ready) .jpg Category Images supplied by you for
your Company Name and The Final Sub Category you Peronally chose.

USADI will supply all other category images as they are our responsibility.
